Ask the Expert - The 'Difficult' Joint Report

In medico-legal practice the joint report process should be, and most of the time tends to be, a straightforward process between the experts instructed by the opposing parties, setting out areas of agreement (often quite a lot), any areas of disagreement, and why the experts disagree on those points, to simplify and narrow the issues for the court. While most of the time, between medico-legal experts well-versed in the process, it is a straightforward matter, on occasion we have all had experiences where the process is more difficult, for a variety of reasons.
A panel of orthopaedic medico-legal experts and Counsel will set out the process and provide tips and strategy for dealing with such scenarios, with opportunity for questions and discussion with the delegates.

How Hand Surgeons Can Foster Enthusiasm and Engagement Across a Whole Career

In line with the overall congress theme this session will explore the different ways hand surgeons and the BSSH are nurturing the profession. From student members and workshops to trainee representatives on committees in the early years we have the Diploma and Mentoring programmes for navigating the transition to consultant practice. Later on different opportunities beckon with multicentre research projects, committee work, work with the Hand Therapists and masses of overseas ventures. There are a many different ways to find new challenges and greater meaning in the subspecialty right up to retirement. In some cases colleagues continue to make significant contributions after retiring from UK clinical practice, benefiting many trainees and colleagues.

Injectables and Cell Therapies for Cartilage Repair/Osteoarthritis: Clinicians and Scientists Working Together to Understand Mechanism and Improve Stratification

This session will explore the current injectable and surgical approaches used in the treatment of cartilage injuries and osteoarthritis, with a particular focus in the knee:
Interventions covered will include hydrogels, blood and bone marrow derived biologics as well as advanced therapy medicinal products (ATMPs), including autologous chondrocyte implantation (ACI).
An understanding of the changing regulatory landscape pertinent to these interventions is key and will be reviewed.
The process and challenges of undertaking a clinical trial to evaluate efficacy will be explored.
Exemplars describing how clinicians and scientists can work together to better understand mechanism and improve stratification of treatments will be presented.

An Introduction to Coronial Law and Mock Coroner's Inquest

Many orthopaedic surgeons will find themselves making an appearance in Coroner's court at some point during their careers. This session sets out to provide some background information as to the role of the Coroner, the nature of proceedings in Coroner's court, dos and don'ts in giving evidence, culminating in a mock Coroner's inquest to observe first hand how the court runs. The aim is to demystify the process and give orthopaedic surgeons some prior understanding and confidence, in case they ever have to attend such proceedings to give evidence.

The British Orthopaedic Oncology Society (BOOS) and the British Elbow and Shoulder Society (BESS) are delighted to host this joint session looking at: Tumour pathology that commonly affects the upper limb
There will be an introductory short talk by Professor Tom Cosker (Oxford) who will give an outline of common primary malignant bone tumours which can affect the upper limb and which might present to the general orthopaedic surgeon in e.g. a fracture clinic.
We will discuss the initial management and investigation of such cases and how a prompt referral can be made to the local tumour service. We will highlight common mimics such as infection / fibrous dysplasia which should also be considered.
The session will then move on to a review of a series of cases and examine how they were managed (the good and the bad!) and what might have been done differently.
There will be ample time for discussion and attendees are also welcome to bring their own cases for discussion.

Delivering Day Surgery Primary Joint Replacement Outside of a Specialised Arthroplasty Hub

Day surgery arthroplasty is very high on the list of GIRFT for NHS Trusts to achieve. There are multiple extremely successful Day Surgery pathways such as that of SWAOC in Exeter, that achieve huge percentages of large joint arthroplasty within the Day Surgery setting. However, not all Trauma and Orthopaedics Units are attached to or have close associations with an arthroplasty hub or isolated Day Surgery Unit.
In this session we hope to discuss developing a pathway for Primary Joint Arthroplasty within a DGH setting. In addition, advice on selecting appropriate patients, encouraging them that this surgery when undertaken in this method is very positive.
A discussion on the Barriers to Day Surgery Arthroplasty, which is not all about the anaesthetic, and how to develop job roles to improve ambulatory pathways.
The team delivering this session have huge experience of delivering both Trauma and Orthopaedic Surgery via Day Surgery and other specialties. There will be opportunities to ask questions throughout and also to an expert panel towards the end of the session.

Metallic particles, such as screws, wires and other foreign metal bodies in soft tissue and bone can sometimes be a real problem. Sometimes these particles get under the skin accidentally, sometimes intentionally, for instance in fracture treatment. The only method generally available for detecting these metallic particles is the X-ray, which can be costly inconvenient and exposes the patient and the personnel to radiation.
GetMet is a perfect solution. It is an extremely handy device, suitable for detecting all types of metal, including titanium and rust proof metals. In first aid it is suitable for removing metal objects, such as bullets, splinters, nails, metal chips and pins from the body. It can also be used in deeper surgery, for instance in carrying out a laparotomy or a thoracotomy.

Robotic Arthroplasty Systems - Current and Future Innovations

Robotic arthroplasty systems represent a noteworthy advancement and certainly a disruption in Orthopaedic Surgery. These systems combine robotics, computer navigation, and advanced imaging technologies to enhance the precision and accuracy of joint replacement surgeries, leading to better patient outcomes, faster recovery times and better long-term survivorship. Robotic systems allow surgeons to create true personalisation in alignment and implant placement. The plans are based on advanced imaging techniques such as CT scans for pre-operative systems whereas intra-operative systems are anatomical landmark data collection based. During surgery, the robot assists the surgeon in executing this plan with a high level of accuracy, guiding the placement of implants, and ensuring optimal alignment and position.
Many robotic systems offer real-time feedback to the surgeon, providing data on bone cuts, implant positioning, and soft tissue balancing throughout the procedure. This feedback allows for adjustments to be made intraoperatively enabling the creation of patient-specific implants tailored to the individual's anatomy. Advanced robotic systems incorporate techniques for assessing and managing soft tissue tension during surgery. This allows surgeons to achieve optimal balance and stability in the joint, which is crucial for long-term implant success and patient satisfaction.
Robotic arthroplasty systems often collect and offer the opportunity to analyse data from each procedure, contributing to a growing database of surgical outcomes. Machine learning algorithms can then be applied to this data to identify trends, refine surgical techniques, and improve patient selection criteria. Some of these systems are becoming smaller and more portable allowing surgeries to be performed in smaller facilities with smart hand-held tools.
The future of this technology is promising. Integration with advanced imaging modalities such as MRI or intraoperative CT could provide even more detailed anatomical information to guide surgical planning and execution. Continued advancements in robotics and artificial intelligence may enable greater levels of automation in joint replacement surgery, with robots performing certain aspects of the procedure autonomously under the supervision of the surgeon.
Future systems may incorporate sensors capable of providing real-time biomechanical feedback, allowing surgeons to assess joint stability and function with greater precision during surgery. By integrating with wearable sensors and digital health platforms, robotic systems could facilitate the development of personalized rehabilitation protocols tailored to each patient's specific surgical outcome and recovery trajectory. Augmented reality (AR) technology could be integrated into robotic systems to provide surgeons with enhanced visualization and guidance during surgery. AR overlays have the potential to display real-time anatomical information directly onto the surgical field, improving spatial awareness and facilitating more precise implant placement.
The robotic arthroplasty systems are ushering in an era of safer, more personalized, and more effective joint replacement surgeries. As technology continues to evolve, it's likely that we'll see continued convergence of robotics, artificial intelligence, leading to improve patient care and outcomes.

In a recent study published by the RCS Annuals in 2023, it was found that burnout among orthopaedic surgeons was prevalent, with 68.3% experiencing moderate levels of burnout and 22.1% experiencing high levels. There is clear evidence that enhancing the psychological well-being of healthcare professionals correlates with enhanced system performance and improved patient outcomes.
Taking a break or sabbatical could be instrumental in improving mental well-being, job satisfaction, and overall workforce productivity. In this session, we will delve into the experiences of orthopaedic surgeons who have taken breaks and engage in discussions about how these periods of time away have impacted their overall professional performance, with the aim of identifying any strategies to reduce burnout within the orthopaedic community.

Interactive session on amputations
Join us for an interactive session on amputations – looking at surgical technique, rehabilitation, prosthetics and advances in osseointegration implants. It is estimated that there are over 50,000 people in the UK living with amputations and whilst the primary indication is usually for vascular disease, there are a number of orthopaedic indications including trauma, infection, congenital deformity and tumour.
Unlike the vascular population, orthopaedic patients are often a younger patient population who have many years ahead of them thus by perfecting surgical technique together with optimising rehabilitation we can achieve better long term outcomes.

Transforming OA Care: Can Orthobiological interventions postpone surgery in younger patients?
How can we address the large treatment gap in OA with the evidence available today? Osteoarthritis (OA) is a complex disease with no current disease-modifying treatments. However, new research is uncovering molecular changes in early-stage OA, guiding better intervention choices. While early OA can often be managed with weight loss and exercise, and severe OA with joint arthroplasty, there remains a treatment gap for moderate OA, especially in younger patients.
This session will delve into the current challenges and gaps in OA treatment, from both conservative and operative perspectives. We'll examine the impact of these gaps on clinical practice and patient outcomes and explore how emerging technologies might bridge them. With the NHS facing a growing waitlist for joint arthroplasty, we'll discuss how to best utilize existing treatments to manage this crisis.

Major Trauma Awareness Week: Beyond clinical care - the true impact of catastrophic injury
Major physical trauma affects thousands of people every year. Every day dedicated teams of professionals work around the clock to save and rebuild lives following major injury. With advances in clinical care more people than ever are surviving their injuries. This is incredible, but can lead to devastating long term consequences for individuals and their families.
As part of UK Major Trauma Awareness Week, Day One Trauma Support is drawing attention to the life changing impact of major trauma by sharing amazing stories of impact, challenge and the road to recovery.
By listening to patients and families experiences, we can better understand the long-term impact of catastrophic injury on people’s lives. We can provide a leading, credible voice to patients through our own evidence and by collaborating with other organisations who are committed to supporting people to rebuild their lives following major trauma.

This is an exciting opportunity to develop your understanding of Non Technical skills and gain an initial exposure to the NOTTs taxonomy. High performance in domains of Non Technical skills have been shown to improve patient outcomes and reduce mortality, and these skills are now far more prominent in the updated surgical curricula.
The aim of the NOTTs project was to develop and test an educational system for assessment and training of non-technical skills in the intra-operative phase of surgery. NOTTs is a behaviour rating system based on a skills taxonomy that allows valid and reliable observation and assessment of four categories of surgeons' non-technical skill: situation awareness, decision making, communication & teamwork, and leadership.
These are the essential non-technical skills surgeons need to perform safely in the operating room and NOTTs allows measurement of several ACGME (Accreditation Council for Graduate Medical Education) competencies, including professionalism, interpersonal and communication skills, and systems-based practice. The skills taxonomy can be used to structure training and assessment in this important area of surgical competence.
The Royal College of Surgeons Edinburgh NOTTs Masterclass is taught around the world. Faculty development has also occurred for groups in North America, East Africa, Japan, Australasia and Malaysia, all of whom now run their own NOTTs courses. "

Clinical Examination Course
The BOA Clinical Examination Course has run for many years. The course is accessible to all levels of trainees from medical student to FRCS. It is not intended to be a substitute for a FRCS revision course but to provide good foundational skills of examination technique.
It is taught by an experienced group of consultants who have a logical and consistent approach to clinical examination techniques. The format includes a morning of lectures covering all regions of the body, followed in the afternoon by a hands-on session where participants rotate in groups of similar skill level. In these tutorial groups there is a demonstration by an experienced consultant followed by practice in pairs of the techniques learnt and opportunities to ask questions.
